No enrollment fee. All memberships include a 3-month initial enrollment period so Dr. Highley can get to know you and build a real care plan. After the initial period, you may cancel anytime with 30-day written notice. Your billing begins on your first visit date. HSA and FSA cards are typically accepted — check with your plan administrator.
A note about Medicare: Community Primary Care is not able to see Medicare patients at this time. If you have Medicare and are interested in joining, please contact us to be added to our waitlist — we'll notify you when this changes.

- Unlimited office visits — no copays, no per-visit fees
- Same-day or next-day appointments for sick and urgent visits
- 30–60 minute appointments
- Direct communication with Dr. Highley (call, text, secure message)
- Annual wellness exams and health screenings
- Chronic disease management (diabetes, hypertension, cholesterol, thyroid, asthma, and more)
- Acute sick visits (infections, injuries, flu, rashes, UTIs, and more)
- Women's health (pap smears, contraceptive management, menopause care)
- Pediatric care (newborn visits, well-child checks, school and sports physicals, developmental screenings)
- Mental and behavioral health (depression, anxiety, ADHD)
- Basic in-office procedures (skin lesion removal, abscess drainage, cryotherapy, laceration repair, skin biopsies)
- Telehealth visits for established patients
- Discounted lab and imaging pricing through partner networks
- In-office medication dispensing at wholesale pricing

Not Included (and How We Help)
Hospital or ER visits
Not covered by membership. We recommend pairing your membership with a health insurance plan or healthshare for hospital and emergency coverage. Dr. Highley coordinates your care during any hospital stays.
Specialist services
Cardiology, dermatology, orthopedics, and other specialties are not provided in-office. Dr. Highley coordinates referrals and follows up on your specialist care.
Surgeries
Surgical procedures are covered through your insurance. Dr. Highley coordinates preoperative and postoperative care.
Lab and imaging costs
Not included in the base membership but significantly discounted through our partner labs. Detailed pricing will be published as partnerships are confirmed.
Vaccines (at launch)
Not available in-office at launch. Dr. Highley arranges vaccinations through community partners and plans to offer them directly in the near future.
IUD insertion and Nexplanon
Referred to a specialist for placement. Dr. Highley coordinates the referral and handles follow-up care.
Lab & Imaging Pricing
Community Primary Care is finalizing partnerships with lab and imaging providers to bring you transparent, discounted pricing. Our goal is to include commonly-ordered basic labs in your membership fee and offer significant discounts on other labs and all imaging. Detailed pricing will be published here as soon as agreements are confirmed.
Payment & Billing
How billing works: Your membership is billed monthly via auto-pay from the card on file. Your first bill is charged on your initial visit date.
HSA/FSA: Most HSA and FSA plans cover direct primary care memberships. HSA and FSA cards are typically accepted as your payment method.
Payment methods: We accept credit cards, debit cards, HSA/FSA cards, and ACH debit. Cash is not accepted for membership payments.
Initial enrollment period: All memberships include a 3-month initial enrollment period. This gives Dr. Highley time to get to know you, review your health history, and build a personalized care plan — the kind of thorough onboarding that makes DPC work.
Cancellation: After your initial 3-month enrollment period, you may cancel anytime with 30-day written notice. No penalties, no surprise fees.
